Family Support
Lyra Group offers support to family members who may have a loved one in a long-term care facility in Maine. We know that being long-distance and trying to support a loved one can be hard. Lyra Group provides the following services for family members:
Support for crisis situations such as when a family member is being discharged from the hospital to a nursing facility or assisted living
Serve as the designated person to attend care conferences and/or medical appointments
Function as a liaison with facility staff
Attend visits on a requested schedule (ex: if family out of state)
Help manage transitions related to health when a loved one needs to move from assisted living to a nursing facility
Assist with estate clean-out management.
